COMMUNITY ACCESS JOB DESCRIPTION 

Support a waiver consumer to become involved in clubs and organizations including recreational, educational, religious, civic and volunteer opportunities with an outcome of less reliance on formal supports and more on natural supports such as neighbors, friends and church members.

EDUCATION

  • Bachelor’s degree in a human services field from an accredited college or university
  • A Bachelor’s degree in any other field from an accredited college or university plus a least one (1) year of experience in the field of intellectual or developmental disability; or...
  • Relevant experience or credentialing that will substitute for the educational requirements on a year for year basis

TRAINING

  • Must have all the required SCL training

QUALIFICATION

  • Individual must be able to travel locally and occasional overnight.
  • Must have good communication skills both written and verbal and perform tasks independently in a suitable time frame.
  • Performs required documentation

PHYSICAL DEMANDS

  • May require travel with occasional overnight stays.
  • Individual must be able to bend, stoop, reach, pull, sit, walk (long periods) and lift up to 25 pounds.
  • Be able to work 50+ hours a week at times as operational demands require. 

Must also have a valid driver's license and insured vehicle.
